Please complete the registration and housing portions of this form and forward it to ASA's Meetings Department. Pay close attention to the payment portion of the form. ASA will make all hotel reservations; however, if you are paying for your housing with a credit card the hotel will process the charge on your credit card, not ASA. Housing deposit is payable to the Hilton Sandestin Beach & Golf Resort if paying by check. Registration fees are payable to ASA. Fees must accompany registration and housing form in order to be processed. Save a copy of registration/housing form and policies for your records. Housing Cancellation Policies: No refunds will be issued if the reservation is canceled less than 72 hours prior to scheduled date and time of arrival. ASA must receive your housing form and deposit no later than February 10, 2003. Reservations made after February 10, 2003, will be processed on rate and space availability. Room Rates: Room rates are $152 per night per room plus 10 percent room taxes and a $8 per room per day resort fee. A deposit of $152 must accompany the housing form in order to guarantee your reservation. If paying by credit card, the Hilton Sandestin Beach & Golf Resort will charge your credit card for one night room deposit. Room categories and nonsmoking room requests are based on availability. Check-in time is 4 p.m., check out time is 11 a.m. All housing requests should be sent to ASA; the hotel will not accept your reservation directly. ASA will acknowledge receipt of your reservation. All changes should be made through ASA. Registration Cancellations: A 75 percent convention registration refund will be processed if cancellation is received in writing and postmarked by January 10, 2003. After January 10, 2003 and before February 10, 2003, a 50 percent refund will be processed. No fees will be refunded after February 10, 2003. All requests for refunds must be made in writing. All refunds will be processed after the Annual Convention. Tour Cancellation Policy: Any attendee canceling tour registration prior to January 27, 2003 will be assessed a $5 cancellation fee. After January 27, 2003, no refunds will be issued. Registration Specifics: This year ASA will not issue event tickets. Each attendee will be given a color-coded Badge Holder to wear for admittance to functions. Badge Holder color will indicate to staff which level each attendee has registered. ASA Member, ASA Spouse/Shop Manager, ASA Tech, Non-member, Non-member Spouse/Shop Manager, and Non-member Tech paid full registration fee includes Welcome Party, General Session, Refreshment Breaks, Meetings, Seminars (based on availability), Seminar Materials, Division Breakouts, Thursday Breakfast, Thursday Lunch, Friday Lunch, Awards/Graduate Dinner and Dessert Reception and Chairman's Reception and Dinner. Food/Beverage tickets will be available to guests of paid full-registration attendees only. Food/Beverage package will include Welcome Party, Thursday Breakfast, Thursday Lunch, Friday Lunch, Awards Dinner & Dessert Reception, Chairman's Reception & Banquet and all Refreshment Breaks. Use attendee number to specify which function each person is attending on front of form. Admittance to all functions is based on availability, and will be booked first come, first serve. Special Services: If you require special services to attend the Annual Convention (special diet, etc.), check the space provided on the registration form. ASA will contact you concerning your needs.
