Please complete the registration and housing portions of this form completely and forward to ASA's Meetings Department. Pay close attention to the payment portion of the form. Registration payment is payable to ASA, housing is payable to the Gaylord Opryland Resort. Fees must accompany registration and housing form in order to be processed. Save a copy of registration/housing form and policies for your records.

Housing Cancellation Policies: No refunds will be issued if the reservation is cancelled less than 72 hours prior to scheduled arrival. ASA must receive your housing form and deposit no later than MARCH 23, 2007. Reservations made after March 23, 2007, will be processed on rate and space availability.

Room Rates: Room rates are $189 per night, per room plus 9.75% sales tax and 5% room tax. A deposit of $189 must accompany the housing form to guarantee your reservation. The Gaylord Opryland Resort, not ASA, will process all room reservations guaranteed by credit card. Room category requests are based on availability. Check-in time is 3:00 p.m. Check-out time is 11:00 a.m. All housing requests should be sent to ASA, the hotel will not accept your reservation directly. ASA will acknowledge receipt of your reservation. All changes should be made through ASA.

Resort Fee: A resort fee of $10 will be charged per room per night and is subject to 9.25% sales tax. This fee includes high speed Internet access, access to Cyber Cafe, local and toll-free calls up to 20 minutes, access to fitness center, USA Today delivered (Monday-Friday) to each guest room, 2 bottles of water provided to each guest room (refreshed daily) and scheduled complex transportation.

Parking: All self-parking is $10 per day. Valet parking is $18 plus 9.25% tax per day.

Shuttle Service: Airport shuttle service available - round trip is $30 per person, one way is $20 per person. Tickets can be purchased at airport kiosk at bottom of escalator or at the hotel from the concierge desks or front desks in the Magnolia and Cascades lobbies.

Registration Cancellations: A 75 percent convention registration refund will be processed if cancellation is received in writing and postmarked by March 23, 2007. After March 23, 2007, and before April 13, 2007, a 50 percent refund will be processed. No fees will be refunded after April 13, 2007. All requests for refunds must be made in writing. All refunds will be processed after the Annual Convention.

Tour Operator Cancellation Policy: Any attendee canceling tour registration prior to March 23, 2007, will be assessed a $20 cancellation fee. After March 23, 2007, no refunds will be issued.

Registration Specifics: Convention Registration includes: Continental Breakfasts, Lunches, Receptions, Celebration of Excellence, Meetings, Seminars and Seminar Materials.

Celebration of Excellence: Tickets will be available to those who choose to only attend the Celebration of Excellence for $75 per person.

Use attendee number to specify which function each person is attending on front of form. Admittance to all functions is based on availability, and will be booked ³first come, first serve.²

Special Services: If you require special services to attend the Annual Convention (special diet, etc.), check the space provided on the registration form. ASA will contact you concerning your needs.
